وقتی از نیم رخ به یک فرد طبیعی نگاه می کنیم ،  چانه مختصری عقب تر از قسمت قدامی لب تحتانی قرار می گیرد . خطی که از نوک بینی به لب تحتانی کشیده می شود باید از برجسته ترین نقطه چانه در نمای نیم رخ بگذرد اگر چانه عقب تر از این میزان باشد اصطلاحا به آن (رتروژن یا نقص عقب بودن چانه ) می گویند که دو علت دارد : یا فک تحتانی به طور کامل کوچکتر و عقب تر از حد طبیعی است که در این موارد رابطه دندانی بیمار مختل میشود یعنی دندان های پایین که به طور طبیعی 2 تا 3 میلی متر بالاتر از دندان های بالایی هستند خیلی عقب تر از دندان های بالایی هستند که در این موارد بیمار باید فک تحتانی را به طور کامل عمل کرده و به سمت عقب بکشد .

در این گونه موارد ابتدا متخصص اورتودنسی بد شکلی های اندامی ناشی از این ناهنجاری را اصلاح میکند ، سپس بیمار تحت عمل جراحی جلو کشیدن فک بالا قرار میگیرد . اما اگر رابطه دندانی بیمار طبیعی باشد محل و اندازه فک تحتانی طبیعی است و قوز چانه . به اندازه کافی رشد نکرده است در این گونه موارد با گذاشتن پروتز که یک جسم تو پر به شکل چانه و از جنس سیلیکون و یا پلی اتیلناست درمان می شود . هر چند این ماده صنامی است ولی بدن وجود آن را به راحتی تحمل میکند و به اصطلاح زیست سازگار است و در 99 درصد بیماران تا آخر عمر بدون هیچ مشکلی در صورت بیمار باقی می ماند . گذاشتن آن بسیار راحت تر از انجام عمل جراحی فک است و دوران نقاهت بعد از عمل نیز استوئوتومی فک (شکستن استخوان فک به منظور اصلاح ناهنجاری ظاهری ) کوتاهتر و ساده تر است ماندگاری پروتزهای چانه مادام العمر است و عموما در دراز مدت جایگاه و شکل خود را حفظ می کند .احتمال عفونت پروتز ها در تمام طول عمر بیمار بین نیم تا یک درصد است و خوشبختانه در صورت بروز عفونت با در آوردن پروتز که عمل راحتی است عفونت به طور کامل بر طرف می شود . تزریق ژل دائمی به منظور برجسته کردن چانه هر چند بسیار ساده است اما با میزان عوارض بالایی همراه است و در صورت بروز عارضه درمان آن دشوار و بعضا غیر ممکن است .
